Self referral forms are available at reception for patients who would like to have their feet checked by our podiatrist. Once the forms have been completed the podiatry department will contact you to arrange an appointment at Blaydon Leisure and Primary Care Centre, Shibdon Road, Blaydon on Tyne. NE21 5NW. Home visits can be arranged for housebound patients.
