Access to your Personal Information
You have a right under the Data Protection Act 2018 and the General Data Protection Regulations to access or view what information the surgery hold about you. You also have the right to have it amended or removed should it be inaccurate. This is known as the “the right of subject access”.
If we do hold information about you, we will:
- Give you a description of it
- Tell you why we are holding it
- Tell you who it could be disclosed to
- Let you have access to it, or a copy of it
If you would like to make a subject access request, please submit your request in writing, or via email to the practice manager. You must include your name, address and date of birth. We will also require two valid forms of identification. If you are not able to make a request in writing, please ask at reception for help and advice.
If you would like access to all the data we hold on you, you may want to consider having online access to your records. You can also indicate this to us on the form overleaf.
Will there be charge for this?
In most cases there will not be a charge. There is no charge for online access.
There are however some circumstances when we will charge for this, for example, repeat requests, or when requests are excessive or unfounded. If a charge is necessary we will inform you of this, and request payment in advance.
